Completing Your New Patient Paperwork
If you're a new patient, the online form above covers your medical and dental history along with the consent items we need on file. Completing it before your appointment means less time at the front desk and more time with your dentist. Prefer paper? You're welcome to fill everything out when you check in at our Albuquerque office instead.
